We know that for many people living with spinal cord injury, having the ability to remain in your own home is an important part in helping you maintain a sense of independence.

With in-home support from our skilled personal care team, you can maintain your lifestyle in the way that you choose. Your support teams take the time to develop a deep understanding of your individual needs, which means we can offer a high level of care with a personal touch.

What Forward can offer you:

  • Specialist care
    We specialise in spinal cord injury (SCI) and similar neurological disability. Our team complete intensive in-house training in all aspects of SCI care: bowel care, bladder care, continence management, safe transfers, use of mobility equipment, pressure area and skin care and identification and response to episodes of autonomic dysreflexia.
  • Planning
    We work together with you to develop and implement your individualised care plan.
  • Collaborative care
    We take a holistic approach to care and work together with you, your family members, physios, OTs and other service providers.
  • Highly qualified support staff
    In addition to your support workers, you will also be supported by a highly qualified team including an assigned rostering lead, client manager, team leader and care coordinator.
  • Flexibility
    We understand that your support needs may change over time, which is why we offer flexible rostering to suit your timetable.
  • 24/7 support
    Our after-hours team are available to assist you outside of business hours.
  • Free PPE
    We provide all PPE for our services at no cost to you – gloves, as well as additional PPE as needed during outbreaks such as COVID.
  • Best practice SCI support
    Our team are supported by Forward’s clinical experts who provide ongoing education in spinal-specific care and best practice guidelines for SCI support. An appointment may be booked with our Clinical Consulting Team for health and wellbeing reviews.

    We have over 35 years of experience supporting people in the community with their personal care needs.

Forward support services include:

  • Bowel care and management
  • Bladder and catheter care
  • Showering, bathing, grooming and dressing
  • Safe transfers, including use of hoists
  • Support for your exercise or physio program
  • Light domestic duties
  • Laundry and cleaning
  • Shopping and preparing meals
  • Assistance with feeding
  • Accessing the community, appointments and social activities
  • Assisting you with transport
  • Support with daily activities

How are services funded?

Forward works with many different funding bodies including NDIS, iCare and other insurance bodies, Disability Supports for Older Australians, Department of Veterans Affairs, private funding and Self-Managed Aged Care. Our prices are based on the NDIS price guide and updated accordingly. Forward is registered for Temporary Transformation Payments (TTP) until 30 June 2022.

Personal care referral process

Referral Form

Once you send us your completed referral form online, we will respond within 2 business days.

Initial Meeting

We will schedule in an appointment to meet with you to discuss about your support needs and our service. We will also complete our assessment forms with you to have a better understanding of your daily life.

Review

Our team will review your referral. We may request for additional information such as medical documents, reports and assessments. We may also request for consultation with your allied health team during this process.

Outcome

Our team will contact you to advise of the outcome of your referral. If we are unable to provide you with services, we will let you know the reason (e.g. rostering capacity, staff not trained in your care procedure etc) and refer you to other providers who may be able to assist you.
